Managing Chronic Conditions Independently
South West Self Management Program is providing a FREE 6 week workshop to St. Thomas Public Library customers!
Begins Thursday, April 3 and runs every Thursday until Thursday, May 8, 2014
1:00 p.m. to 3:30 p.m. – Board Room, second level

Having the confidence to manage all aspects of your life including daily activities and your medical needs – we call these skills “self-management skills”. This program is a self-management concept for persons living with chronic conditions. Participants discuss how their condition affects their life and how they are currently managing it. They are then assisted with setting realistic goals and building confidence in managing their condition.
This free and effective 6 week self-management program developed and researched at Stanford University is now offered around the world and St. Thomas residents have a fantastic opportunity to join this workshop!
You should attend this workshop if:
- You are someone with a chronic health condition such as high blood pressure, arthritis, heart disease, stroke, Parkinson’s disease, diabetes, mental health issue, asthma/COPD, osteoporosis, obesity, etc.
- You are interested in learning how to better manage your chronic condition
- You are caring for someone living with a chronic condition
Participants will receive a free copy of the book “Living the Healthy Life with Chronic Conditions.” ($18 value and available in audiobook format for those who need it.) This program is offered in partnership by the South West Community Care Access Centre and South West LHIN.
